                                                CLINIC DENTIST

JOB CLASS:   EXEMPT

JOB RESPONSIBILITIES:

Under the supervision of the Dental Director, performs professional dental services in the examination, diagnosis, and treatment of patients and performs related duties as assigned by the Dental Director.

DUTIES PERFORMED:

           Assist Dental Director in the development of guidelines for dental treatment and

            in the establishment of priorities for dental treatment.

           Advises the division head or project director in the development and

            implementation of dental policies.

           Examines patients accepted for dental treatment.

           Diagnoses dental disease for admitted patients; establishes the plan for treatment

            and prevention of dental disease.

•           Provides treatment for diagnosed dental disease and/or refers patients to the

            appropriate agency or professional individual when indicated.

           Advised patients in the prevention of dental diseases.

          Performs other clinical duties as assigned by Dental Director.

•           Performs quality assurance audits and peer review as assigned.

           Provide dental education to clinic clients, in individual or group setting as

            assigned by Dental Director.

           Maintain accurate, legible and complete patient records.

           Share on-call duties for dental department.

KNOWLEDGE, SKILLS, AND ABILITIES:

           Thorough knowledge and ability in the art of dental examination, diagnosis,

            and treatment.

•           Thorough knowledge of the principles of dental disease prevention.

•           Considerable knowledge of the principles of dental health education.

           Considerable ability to deal tactfully and efficiently with patient and staff.

TYPICAL WORKING CONDITIONS:

           Light lifting.

           May require prolonged standing during dental procedures.

           Transfer of wheelchair patients with assistance.

           Use of CRT/PC

MINIMUM QUALIFICATIONS:

Graduation from an accredited dental school with a D.D.S., D.M.D., or equivalent degree and currently licensed to practice dentistry in the State of Minnesota. (No substitution for education or license requirements).

            

                       JOB TITLE:  PHYSICIAN – FAMILY PHYSICIAN, OB PREFERRED

JOB CLASS:  EXEMPT

JOB RESPONSIBILITIES: 

Provides clinical services to patients of Open Cities Health Center, Inc., and may be involved in other duties as assigned by the Medical Director.

DUTIES:

·               Provides outpatient care during a 5-day workweek.  Clinics are scheduled for 4 ½ days a week for a full time position.

·               Develops a panel of continuity patients as well as seeing walk-in and urgent care patients as the clinic needs.

·               Participates in quality assurance peer review auditing activities and other performance improvement activities as assigned by the Medical Director.

·               Participates in other committees and clinical activities as determined by the Medical Director.

·               Participates in Continuing Medical Education activities.

·               OB providers will participate in a shared call schedule with midwives and family physicians at Regions Hospital. Time spent in the hospital for rounds and call are compensated from clinic hours. 

·               Non-OB providers will participate in a telephone-only after-hours call schedule with no inpatient coverage. Non-OB providers rotate through a Saturday clinic schedule and are compensated from the week-day clinic for this time.

 JOB TITLE:  CLINIC OPTOMETRIST

JOB CLASS:  EXEMPT

JOB RESPONSIBILITIES:

Under the supervision of the Medical Director, diagnose, manage, and treat conditions and diseases of the human eye and visual system. Examine eyes and visual system, diagnose problems or impairments, prescribe corrective lenses, and provide treatment. May prescribe therapeutic drugs to treat specific eye conditions.

DUTIES:

·         Examines eyes and vision

·         With or without the use of diagnostic drugs, examines eye for presence or absence of defects or visual anomalies that can be corrected, remedied, or relieved by using lenses, prisms, orthopedics, contact lenses, or other ophthalmic devices.

·         Performs external and internal examination of eyes and refers pathology to medical specialties.

·         Determines refractive errors such as myopia, hyperopia, or astigmatism, and prescribes treatment for these and other visual problems.

·         Ascertains, range of vision and ability and ease with which eyes maintain clear, comfortable, binocular vision at any distance of fixation.

·         Examines eyes with instrumentation and observation to determine visual efficiency and performance.

·         Prescribes corrective procedures

§          Provides outpatient care during nine one-half day clinics. Emphasis is placed on the doctor-patient relationship.

§          Participates in Quality Assurance peer review auditing activities and other QA activities as assigned by the Medical Director.

§          Participates in other committees and clinical activities as determined by the Medical Director.

·         Participates in Continuing Education activities, and obtain at least the minimum hours as required by the State licensing requirements. Additional hours are encouraged.

·         Provides assistance, as needed, to the Medical Director on visual issues pertinent to the clinic and contributes as needed to the development of the annual Health Care Plan.

·         Performs other duties applicable to the position, as assigned.

§           Strong personal and professional communication skills.

§           Knowledge of and desire to work within a Public Health/Community Medicine model of service delivery.

     MINIMUM QUALIFICATIONS:

·         Licensed Optometrist

·         Doctor of Optometry Degree from an accredited optometry school

·         Passed the National Board examination and a written national, regional or state clinical examination

·         Maintain continuing education credits

·         Current Minnesota Licensure

·         Eligible for Credentialing with Most Major Insurance Providers

·         Strong community Health/Public Health orientation.

CLINIC PHYSICIAN ( FAMILY PRACTITIONER)

JOB CLASS:  EXEMPT

JOB RESPONSIBILITIES:

Under the supervision of the Medical Director, provides clinical services to patients of Open Cities Health Center, Inc., and may be involved in other duties as assigned by the Medical Director.

DUTIES:

§          Provides outpatient care during nine one-half day clinics. Emphasis is placed on the doctor-patient relationship to be the primary physician for the patients. On an urgent basis patients may be seen by providers other than the primary provider.

§          Provides inpatient services as needed, or arranges for appropriate inpatient care at Regions Hospital, St. Joseph’s Hospital, or other local hospitals if sufficient call-sharing for inpatient services is not available within OCHC.

§          Participates in Quality Assurance peer review auditing activities and other QA activities as assigned by the Medical Director.

§          Participates in other committees and clinical activities as determined by the Medical Director.

§          Participates in Continuing Medical Education activities, and obtain at least the minimum hours as required by the State licensing requirements. Additional hours are encouraged.

§           Provides assistance, as needed, to the Medical Director on epidemiological issues pertinent to the clinic and contributes as needed to the development of the annual Health Care Plan.

 Psychiatric Nurse Practitioner (CNS or ANP)

Open Cities Health Center of St. Paul (OCHC), a federally-qualified clinic providing primary care in medicine, mental health and dentistry, is looking for a part to full-time mental health provider to join its integrated care team.  OCHC receives federal funding to serve as a safety net for the underserved and under-insured. Our culturally rich patient population is 70% African-American, Hmong and East African. Many of our patients have experienced trauma. Many present a challenging combination of medical, mental health, and substance use conditions.

OCHC presents an exciting opportunity for the psychiatric nurse practitioner who enjoys challenges, is committed to community health, likes to work as part of a multi-disciplinary mental health team, takes a holistic approach to patient care, would enjoy collaborating with and consulting to medical providers, and likes variety in job responsibilities. These include:

  • Psychiatric evaluation

  • Medication management, in collaboration with an off-site psychiatrist

  • Some crisis assessment and planning (daytime only)

  • Consultation to medical and mental health providers

  • Triage and referral to secondary mental health/chemical healthcare agencies

  • Documentation in electronic health record

  • Developing innovative care delivery methods as needs arise
